Annual Wellness Exams

Patients need to be seen regularly to ensure they are staying in a healthy posture. Annual Wellness Exams are just the beginning of the annual visit and screening process. Many of the events surrounding the Annual Wellness Exam can be completed before the provider’s clinical work. This saves time and directs efforts to those resources that are most efficient. Let’s explore how MasterCareHS can assist with your patients’ Annual Wellness Visits to keep your population intact and healthy.

Patient Engagement

In today’s highly competitive healthcare market, our ability to focus on patients to ensure they get into your clinic for annual exams and gap closure in value-based shared savings programs requires strong engagement methods and a knowledgeable support staff. Let us increase your office traffic when resources are limited and needed on a part-time basis.

Cognitive Testing

The Centers for Medicare Services (CMS) has defined several new Cognitive Testing procedures to pro-actively screen patients for cognitive issues that we view as part of the Annual Wellness Exam process. Once your patient has been qualified for Cognitive Testing, the MasterCareHS team can perform the testing in the home or at the clinic saving time and resources for the analysis and patient-specific care-plan creation. Cognitive Testing can add up to $1,500 in annual reimbursements depending on a patient’s condition.

Health Risk Assessments

Part of the Annual Wellness Exam process requires completion of a Health Risk Assessment (HRA) questionnaire. This is used as part of the Medicare Annual Wellness Visit to identify issues important to an individual’s health and well-being. Our team can complete these assessments remotely before a clinical visit to save valuable provider and staff time and makes the AWV event much more efficient.
